What we do with the HNA

The Darling Downs and West Moreton PHN Health Needs Assessment (HNA) identifies the health priorities and opportunities for our region. Understanding the health needs of people in our community, as well as the workforce and services that provide health care, is a vital first step to improving health outcomes. The HNA helps us to best manage health resources for our region to improve the health outcomes of our community.


By bringing together information from the community, local health services and the workforce, the PHN has developed a clear picture of how we can make a difference. This includes programs and services that can provide people with the right care, in the right place, at the right time.


Our community-driven priorities for the region have been identified through a rigorous process of research and consultation, focused around the seven national PHN priority areas: Aboriginal and Torres Strait Islander health, aged care, alcohol and other drugs, digital health, health workforce, mental health and population health.

We produced the first HNA for the Darling Downs and West Moreton region in 2017. Health is a

dynamic field. It continually evolves to incorporate new evidence-based interventions and new models of care, and the needs of a growing and changing population.


We regularly update our assessment with new data and analysis, and information gained from engagement with our stakeholders, services and the community.
